Acknowledgement of Risk

Participants understand that participating in Reformer Pilates classes involved physical activity that carries inherent risks, including but not limited to muscle strain, sprains, falls, equipment related injuries and other potential physical harm. Participants acknowledge that:

• Pilates exercise requires physical exertion that may cause injury,
• The use of reformer machines and other equipment involves risk of malfunction, misuse or accidents,
• Participants are voluntarily participating and assume all risks associated with Pilates practice. 

Health and Medical Disclaimer

• Participants certify that they are physically fit and have no medical conditions that prevents them from safely participating,
• Participants understand it is their responsibility to consult a physician before beginning any exercise program
• Participants will immediately inform the instructor if they experience dizziness, pain or discomfort during a class.

Release of Liability

• Participants herby release, waive and discharge Tru Kore Studios, its owners, instructors and employees from any and all claims, liabilities, injuries, damages, or legal actions arising from:
• Their participation in Reformer Pilates Classes or any related activity,
• Any injuries sustained on studio premises, whether caused by negligence or otherwise,
• The use of studio equipment, including but not limited to reformer machines, mats, straps and other accessories.

Indemnification

Participants agree to hold harmless and indemnify the studio, its owners, instructors, and staff from any costs, legal fees or claims brought by third parties as a result of my participation.

Photography

I give my permission for the studio to use phots/videos of me for marketing purposes.

Agreement

By creating an account participants confirm they have read and fully understood this waiver and all other policies and procedures and voluntarily agree to it and acknowledge they are waiving certain legal rights.
